DENIAL CODE -18
If very same claim submitted twice or more for exact same date of service, then intial claim will be processed and other claims will be rejected as replicate.

If claim filed twice then we require to act on initial claim. Need to inspect claim status of initial claim through insurance coverage agent or through web website.

If claim submitted as soon as and denied as duplicate, then contact insurance coverage agent and ask for reprocess of the claim as claim filed just once.

If two services are rendered on very same date of service and claim not submitted with proper modifier. Then claim may deny as duplicate.
In this case, we need to add proper modifier and send remedied claim.

Additional points:.
If patient market modifications made on claim, we require to send fresh claim.

If any coding corrections made on claim, require to submit remedied claim with resubmission code 7 and original claim number on corrected claim.
